Your Healthcare Records

At The Rotunda Hospital, you have the right to access your personal information held by us. You can request your data through one of the following methods — please choose only one to ensure efficient processing.

Routine Access – Subject Access Request (SAR)

What This Means:

Routine Access is a simplified process for patients and service users requesting their own personal records. It falls under the wider scope of data protection legislation.

How to Apply:

  • Complete the Routine Access Request Form – SARS Form.
  • Provide valid photo ID to confirm your identity.
  • Sign the request form.

Response Time:

  • You will receive a response within 30 calendar days from receipt of a valid request.
  • Records are sent via encrypted email or secure password-protected link, unless you specifically request hard copy delivery by post.

Freedom of Information (FOI) Act 2014

What This Means:

The FOI Act allows you to request:

  • Access to your personal and/or non-personal records.
  • Correction or update of personal information that is incomplete, incorrect, or misleading.
  • Access to official records held by the hospital.

How to Apply:

  • Complete the FOI Request Form.
  • Indicate whether your request is for personal or non-personal information.
  • Provide ID if required to confirm your identity.
  • Sign the request form.

Response Time:

  • A decision will be issued within 20 working days.
  • Records are sent via encrypted email or secure password-protected link, unless hard copy delivery is requested.

General Data Protection Regulation (GDPR) 2018

What This Means:

Under GDPR and Irish Data Protection law, you have the right to request a copy of your personal data held both electronically and in manual records.

How to Apply:

  • Complete the GDPR Form.
  • Provide valid photo ID if requested.
  • Sign the request form

Response Time:

  • A response will be issued within 30 calendar days of receiving a valid request.

Records are sent via encrypted email or secure password-protected link, unless hard copy delivery is requested.

Freedom of Information Act

Accessing Records of the Deceased

The Rotunda Hospital believes that all patient information has been provided to us confidentially and you may not be given access to another person’s records.  Applications for the personal information of someone who is deceased are only processed under the Freedom of Information Act. Each request is considered based on the information provided.

The death certificate of the deceased must accompany the request along with proof of your relationship.

Fees:

Fees may apply for some personal records, for example copies of x-rays.  Once we commence working on your request we will be in contact with you regarding any fees that may apply.

How long will the request take?

Your request should be processed within 20 working days.  However, there are some circumstances where it may take longer.  We will advise you of this and of your rights of appeal under the legislation.

Freedom of Information & Data Protection Law

The Freedom of Information (FOI) Act allows for members of the public to make applications for their own personal records, the records of others and the corporate documents of the organisation.

It also allows for records to be amended and for you to have reasons for decisions explained to you.

Data protection law ensures that we will only use the personal data and information you give to us or that is created while you are attending the hospital only for the purpose that you intended i.e. providing you with the best care while you are pregnant and following the birth of your baby. We are required to keep your records safe, accurate and up to date.
